The therapist must comply with the policies on privacy, confidentiality and security as outlined by the Privacy Standard by the College of Physiotherapists of Ontario. All patient personal health information that employees have access to or learn through employment or affiliation with Julie Gray Physiotherapy is confidential and that under the Personal Health Information Protection Act (PHIPA). All Employees have a legal obligation to protect the privacy of patient health information. All staff, Human Resource and payroll information that therapists have access to or learn through employment or affiliation with Julie Gray Physiotherapy is confidential and all therapists have an obligation to protect that information. As a condition of employment or affiliation with Julie Gray Physiotherapy, therapists must comply with these policies and procedures.
Therapists must not access, use or disclose any confidential and/or patient personal health information that they learn of or possess because of affiliation of Julie Gray Physiotherapy, unless it is necessary to do so in order to perform job responsibilities. Under no circumstances may confidential and/or patient personal health information be communicated either within or outside of Julie Gray Physiotherapy except to other persons who are authorized by Julie Gray Physiotherapy to receive such information. Therapist cannot alter, destroy, copy or interfere with this information, except with authorization and in accordance with the policies and procedures and my duties and responsibilities. Therapist must keep any computer access codes (for example, passwords) confidential and secure. Therapist will protect physical access devices (for example, phone and laptops) and the confidentiality of any information being accessed. Therapist cannot lend access codes to anyone. These access codes come with legal responsibilities and the therapist is accountable for all work performed under these codes. If the therapist has reason to believe that access codes or devices have been compromised or stolen, they must immediately investigate the situation.
Charts must be kept and destroyed as per the College of Physiotherapists of Ontario Guidelines. Please refer to their position statement in the CPO binder for full details. All therapist charts must be kept onsite at all times. No part of a chart can be copied or mailed/faxed out without the client’s written permission. Only the treating therapist can read the patient’s chart. All charts must be kept within the charting cabinet or the treating therapist’s possession in the clinic, they are not to be left within reach of other patients or staff or in common areas. Charting cabinet must be locked each night by the physiotherapist. Only the physiotherapist, Julie Gray, can have a key to the filing cabinet. No other copies of the key can be made. Charts of discharged clients must be kept for 10 years for adults and 10 years after turning 18 for peadiatrics. Patients, visitors or other personnel are never allowed access to the charts. Charts will be destroyed after the given timelines as per CPO guidelines by Julie Gray.
